Billing & Insurance Information
Our hospitals, outpatient locations and physician practices welcome most insurance plans. If you have a participating health insurance plan and provide us with your policy information, we will file your insurance claims for you. Insurance coverage, co-pays and deductibles for office visits and inpatient and outpatient procedures are determined by your insurance plan and are expected at each visit. Please check with your insurance company to understand your coverage and your responsibilities. Please advise us of any changes to your insurance information or mailing address.
You may contact customer service to discuss available payment options. Please be sure to bring your insurance card to your appointment.
If you do not have health care coverage you will be asked to pay a portion of your estimated charges at the time of service. You may contact customer service to discuss available payment options.
If you do not have health care coverage and cannot afford to pay for needed services, please contact our financial advocates to discuss our financial assistance policy.

To make a payment in person, just visit a cashier's offices at any of our hospital locations.

FORMS OF PAYMENT:
If you provide insurance information, we will file your insurance claim for you. Once your insurance company has paid, you will receive a bill that will reflect any remaining deductible, co-payment or patient responsibilities. We accept the following forms of payment:

- Bank Debit (Payment Plans)
FINANCIAL ASSISTANCE PROGRAM:
If you do not have health care coverage you will be asked to pay the full amount of your estimated charges at the time of service. If you cannot pay the entire estimated amount, we will ask you to speak to a customer service representative at 904.202.2092, option #3.
