Baptist AgeWell Center for Senior Health is a specialized outpatient center and care team focused on promoting healthy aging for older adults with complex needs. The center is designed to serve patients 65 and older who have experienced an unexplained decline in health and/or have had two or more hospital visits in the past twelve months. We offer a comprehensive approach that looks at the whole person – and it all starts with your personalized geriatric assessment.
First Visit - Geriatric Assessment
This is the “getting to know you” visit to determine the patient’s current health, functioning, and goals for daily living. Patients are encouraged to bring their caregiver or a family member, and all current prescribed and over-the-counter medications. The first visit typically lasts 3 hours.
Second Visit - Personalized Care Plan
In preparation for this visit, the geriatrician, psychiatrist, care coordinators, pharmacist, therapists, dietician and nurses collaborate to create a plan of care designed to support each patent’s health, independence and personal goals.
AgeWell Emergency Care
Our innovative service for older patients is located at Baptist Medical Center Jacksonville and includes a specialized team focused on geriatric issues, in an environment that acknowledges and supports your needs.
Advocacy and Education
Our eldercare advocacy program help seniors with complex medical needs have a smooth transition from hospital to home. Our ageless wisdom training program sensitizes caregivers on what it’s like to be an older patient. Participants learn first-hand about visual limitations, reduced hearing, and breathing issues that can impact seniors’ ability to distinguish among pills and to understand instructions from physicians.
