Job Summary

Baptist Health is hiring a Coordinator, Clinical (RN) Corp. to join the P.A.T.H. team.  This is a PRN position.  This position is primarily for Baptist Nassau but might occasionally be at other Baptist locations. 

PRN (contract work on an as-needed basis) – Position is not full/part-time
 
In 2014, Baptist Health established PATH (Personalized Approach to Health) to support employers in improving the health and well-being of their employees. The RN Clinical Coordinator has dual responsibilities for supporting corporate client employee wellness programs and coaching employee participants on how to improve their health and well-being.

Job Description:

  • Serves as program coordinator for corporate health initiatives such as annual physical programs
  • Places lab orders and completes administrative tasks to support annual physical program
  • Attends biometric screening events on-site at employer clients’ offices to provide oversight and guidance to participants who have critically high biometric screening values (blood pressure, glucose, A1c)
  • Provides health coaching to participants to help improve their lifestyle habits such as nutrition, exercise, stress management, sleep, medication adherence, etc.
  • Identifies possible co-morbidities in participants and evaluates as indicated. Identifies absolute and relative contraindications. Communicates all findings with healthcare team and participant
  • Coordinates follow-up care as appropriate for each participant
  • Presents health and wellness workshops on-site and virtually for corporate clients. Topics may include education around blood pressure, glucose, cholesterol, chronic disease prevention, injury prevention, etc. (Optional)
  • Maintains and submits accurate data for employer reporting and regulatory agencies in order to ensure compliance with reporting requirements and proper quality outcomes monitoring.

Candidate Qualifications:
Must be willing to travel locally throughout the greater Jacksonville area to employer clients’ offices. Bonus if willing to travel outside of Jacksonville occasionally (less than 10%).

  • Must be willing to support on-site programs at Baptist Nassau hospital once per week, 3 months of the year

Education Required

Bachelor's Degree - Nursing

Education Preferred

Master's Degree - Nursing

Experience

  • 3-5 Years Clinical Experience Required

License and Certification

  • Basic Life Support (BLS) Required
  • Health Coach Certification Preferred
  • Licensed Advanced Registered Nurse Practitioner Required Or
  • Licensed Registered Nurse Required Or
  • Licensed Physician Assistant Required Or
