Baptist Heart Specialists is seeking a CHF Clinical Coordinator (Nurse Practitioner or Physician Assistant) to join our growing practice at Baptist Medical Center South. The CHF Clinical Coordinator will coordinate the patient’s care to increase the efficiency and quality of care that is delivered to the patient. Position includes the major responsibilities of the essential job duties and to extend the responsibilities beyond the episode of care to encompass care coordination with hospital collaboration. CHF Clinical Coordinator will be the patient advocate and care for them in a highly supportive and personal manner and also will serve as a liaison to ensure that patients and families receive resources and services as needed. The CHF Clinical Coordinator will be a link between inpatient and outpatient care.
- Master of Science, required
- Certified Physician Assistant (PA-C) or Nurse Practitioner
- Licensed Physician Assistant or Nurse Practitioner
- Cardiac Critical Care Experience, Preferred
