We currently have this unique opportunity for a Full-time Post-Acute Advanced Practice Registered Nurse, who is interested in and passionate about skilled nursing. This position works as part of the team at River Garden Senior Services at River Garden Hebrew Home for the Aged. We are seeking a Nurse Practitioner or Physician Assistant to join our Post-Acute team at River Garden. The ideal candidate will have practice experience in post-acute programs and comfortable with Palliative Care and End of Life Care Measures. Our physicians provide safe and effective model of care while transitioning the patient from inpatient to outpatient primary care.
