Our patient access specialists are dedicated to helping you plan and schedule your first appointment. We know how important it is to ensure your clinical team has everything they need in a timely manner in order to develop your care plan. We are always here to help.
Scheduling Your First Appointment
Please call our dedicated patient access team at 1-844-632-2278 or fill out an online form to request your first appointment. Our goal is to make appointment scheduling as easy as possible.
We will be happy to verify your insurance coverage and work with you to get relevant medical records transferred before you arrive. You will receive detailed information about your appointment by the end of the next business day.
If you will be coming from out of town for your first visit, please try to keep your travel plans flexible. Your initial visit may take several days while tests are completed and a care plan is developed. The length of stay depends on your treatment plan. If you would like a list of nearby hotels, let us know.
Planning for Your First Visit
You will begin your first visit by registering with your patient access specialist, who will review your insurance benefits, make sure your information correctly entered in our system, and confirm that all required forms are completed. We ask that you arrive at your scheduled time for your registration appointment, as this process may take up to one hour. This will be followed by your appointment with the doctor.
When you arrive for your first appointment with us, we want you to feel confident and relaxed. We’ve listed some planning tips below to ensure we can make your first visit as easy as possible.
Gather Your Information
Your patient access specialist will request specific information from you based on your diagnosis and any prior treatments. Please bring this information with you to your first appointment.
To help expedite your first visit, we provide our health forms online (on this page) so that you can print them out and complete them at your convenience. If you have any questions, please don’t hesitate to contact us.
Any medical information you have been asked to bring (may include diagnostic images such as CT scans, MRIs, PET scans, ultrasounds, X-rays, pathology slides and reports).
A list of all your doctors with whom you would like us to communicate. Please include the doctor’s full name, address, phone number and email address, if possible.
A list of all prescription and non-prescription medications you are taking, including vitamins, over-the-counter drugs, and nutritional or herbal supplements.
A list of any known allergies.
What to Bring with You
We encourage you to wear comfortable clothing and shoes and bring something to keep you warm, such as a sweater, jacket, small blanket or shawl. In addition, please bring:
Valid photo ID such as driver’s license or passport – this is critical for the registration process. We cannot register you without a valid photo ID.
All health insurance and prescription insurance cards.
You may also want to consider bringing your appointment schedule and any other materials we have provided, including driving and parking directions.
In addition, you may want to bring a small notebook for writing notes and/or a laptop or mobile device. You can access our free wireless network within the center.
After registration, you will meet a member of your heath care team. If you need to have a test (such as a giving a blood sample or having an imaging test), this will happen before your physical exam.
Your doctor and care team will meet with you and review your medical history and any test results and perform a physical exam. Afterward, the medical team will review treatment plan options with you and your family and we will work with you to schedule any necessary follow-up appointments.
