Please follow these billing instructions for Baptist Primary Care services that were provided at an office visit or for services performed while a patient at a Baptist Health hospital or outpatient facility.

When your statement of benefits arrives from your health insurance company, please verify that payment has been made to Baptist Primary Care. Ultimately, payment responsibility rests with the patient.
