Job Summary
Baptist Health is seeking a dedicated and compassionate Advanced Practice Provider (APP) to join our team in a remote capacity supporting patients across Jacksonville, Florida. In this role, you will provide high-quality, patient-centered care by evaluating, diagnosing, and managing a variety of health conditions.
The APP will collaborate with a multidisciplinary team to deliver evidence-based care, improve patient outcomes, and support Baptist Health’s mission of providing exceptional healthcare to the communities we serve.
What You’ll Do
Conduct comprehensive patient assessments, including medical history and physical examinations
Order and interpret diagnostic tests and lab results
Develop and implement treatment plans, including prescribing medications
Provide patient education on disease prevention, wellness, and treatment options
Collaborate with physicians and care teams to ensure coordinated care
Document patient encounters accurately in the electronic medical record (Epic)
Participate in quality improvement initiatives and clinical best practices
Qualifications
Master’s degree in Nursing (MSN) or related advanced practice degree
Certified Nurse Practitioner (AANP or ANCC)
Active Florida APRN license (or eligibility)
DEA Controlled Substances Registration
Current, valid driver’s license
Strong clinical judgment and communication skills
Experience in telehealth or remote care preferred
