To safeguard your personal health data, My Baptist Chart now requires two factor authentication (sometimes referred to as two step verification) to access your account. Two factor authentication helps keep your account safe by asking for your password plus a special code that we send to your phone or email. This way, only you can get into your account, even if someone else knows your password.

How often do I need to use two factor authentication to access MyChart?
You'll need to use two-step verification the first time you sign in on any new device. But you can check the box that says "Skip this step next time" so you won't have to do it again on that device for 6 months.

Is two factor authentication required?
Yes, it's required. An alternative to this process is setting up a passkey, which uses your device's built-in security features like fingerprint scanner or face recognition to sign you in. Learn more in the "What is Passkey?" FAQ below.

I didn't receive my 2-step verification code.

• Try checking the spam or junk folders in your email. If the email with your code is not there, try clicking Resend Code.
• If you still didn’t receive the email, it could be that your account has a different email address on file. If you have multiple email accounts, check one of the others to see if the verification code was sent there instead.
• If none of your accounts received the email, it could be that we don’t have an email address on file. If this is the case, fill out this form and one of our team members will assist you.

A passkey is a more secure alternative to traditional passwords. It uses your device's built-in security features like fingerprint scanner or face recognition to sign you in to My Baptist Chart quickly and securely. We're introducing passkeys to make your My Baptist Chart access both more secure and more convenient. They're backed by major technology companies like Apple, Google, and Microsoft as the future of online security.

Do I have to use a passkey?
No, traditional password login will still work, but you will be required to use two-factor authentication (you receive a text/call with a code to enter). We recommend setting up a passkey for your convenience and enhanced security.

How do I set up a passkey for My Baptist Chart?
When you log in to My Baptist Chart, you'll see a message asking if you'd like to create a passkey. Just follow the prompts given.

How do I remove or change a passkey?
To remove a passkey, go to the Passkey Management page within Account Settings and click the Remove passkey button.

We aim to protect your privacy and information. If you remain idle for 10 minutes or more after you log in to My Baptist Chart, you will be automatically logged out. We recommend that you log out of My Baptist Chart if you need to leave your computer for even a short period of time.

MyChart Central allows you to view and manage all of your MyChart accounts with one login.

Here's how to sign up:

• Log in to My Baptist Chart and follow the prompt to sign up for MyChart Central.
• After accepting the terms, you'll be asked to create an Epic ID—a single, unified login that consists of your email address and a secure password.
• Once you create your Epic ID, you'll see a list of all the MyChart organizations available to link to your MyChart Central account. This may include local organizations such as Nemours, Memorial Healthcare, and others.
• Moving forward, when you log in to MyBaptistChart using your EpicID, you'll be able to quickly access and manage all of your MyChart accounts in one place.

Yes. Virtual visits can only be completed when you are physically located in a state where your provider is licensed to practice. You'll be asked to confirm your location before your appointment. If you're outside an approved state, your visit cannot be completed. Your doctor’s office can discuss alternative options.

There are several different methods of My Baptist Chart signup:

• Clinic staff might sign you up directly while you're at the front desk or in the exam room.
• You may receive a My Baptist Chart activation code on your After Visit Summary or on a billing statement.
• You may receive a text message or email with an activation code when you come in for a visit.

No activation code?
No problem. You can still easily self-signup online to create a My Baptist Chart account. You must be 18 years old and request an activation code. Follow these easy steps to request a code:

1. Scroll up to the Sign In section at the top of this page and click "Sign up now"
2. Click "Sign up online"
3. Click "Match Yourself with Our Records" or "Verify with a Third Party" to supply personal verification items, such as the last four digits of your Social Security number and your date of birth, and have an activation code sent to your email address or mobile phone.

If you don’t have an activation code, you can request one on the sign-up page.

Baptist Health offers a virtual assistant to make scheduling easier. If you receive a text from the code 27068, your virtual assistant can help you reschedule an upcoming appointment, cancel or find a new one. Just follow the prompts and you will be able to reply with the days and times that work best for you, and your assistant will show you available options so you can book right from your phone. It's a secure service from Baptist Health, designed to save you time.

You can also log in to My Baptist Chart any time to confirm or manage your appointments, and you can still call to speak to a team member.

In My Baptist Chart, navigate to Main Menu > Account Settings > Communication Preferences > Details. Select "Advanced settings" in each section for even more options.

Whether it's text, email, app notifications, phone or mail, you can tell us how you want to hear from us on a variety of topics.

In My Baptist Chart, navigate to Main Menu > Account Settings > Personal Information > Contact Information > Edit. Once you have updated the mobile phone number, select Save Changes.

If you opted in with a mobile phone number that is the same number listed on your proxy account, both accounts will be enabled. Communication preferences can be set individually, or within your My Baptist Chart account at Main Menu > Account Settings > Communication Preferences, then select the check box labeled "Update my preferences for communication I receive about others."

If you already opted in to receive billing text notifications in My Baptist Chart, just follow the instructions in the text message to charge your card on file.

How to Set up Text Payments
The next time you pay a bill through My Baptist Chart, click "Save Payment Method to My Wallet." Also, make sure you're opted in to billing text notifications: In My Baptist Chart, navigate to Main Menu > Account Settings > Communication Preferences > Details > Billing > Advanced settings. Turn on Text message for Statement and Payment Reminder.

In My Baptist Chart, navigate to Main Menu > Billing Summary. Click on Paperless Preferences at the bottom of the page to see your current settings. On the Paperless Preferences page you can turn paperless billing on or off.

To access a minor's or dependent adult's records, you can request proxy access. You must be at least 18 years old to have proxy access to another person’s medical record.

To access another adult's records (spouse, aging parent, etc.), they need to grant you access and give permission. Have them visit the Sharing Hub in My Baptist Chart to invite you.

You may want to grant a family member or friend access to your medical records when you need help managing your appointments or other medical needs. To give someone access to your medical record, visit the Sharing Hub in My Baptist Chart where you can choose what level of access you'd like them to have.

Proxy access allows parents, legal guardians, caregivers and loved ones to access the health information of someone they care for.

No, due to the sensitive nature of medical information, each person must have their own My Baptist Chart account. They can grant you access to their account in the Sharing Hub in My Baptist Chart.

To see your child's bills, you must be listed as the guarantor. Contact your provider to gain access.

Whether you need all or part of your records, you can retrieve them for yourself and securely share them easily using My Baptist Chart. Once signed in, access medical records in the Sharing Hub of the main menu.

With My Baptist Chart, you can access your visit details, lab results, and medical history in one place, as soon as they become available, which may be before your doctor has a chance to review them. It’s always your choice to view these results now or wait to discuss with your care team. You can also adjust your Test Result Preferences on the Test Results page so most future results are postponed until your provider has had time to review them (or for 4 days, whichever comes first).

After your provider reviews your results, you might see additional comments and interpretation in My Baptist Chart.

Important things to consider when viewing test results in My Baptist Chart.

Your provider will review your medical record prior to any upcoming appointments and will populate medications, immunizations and any known health conditions to your My Baptist Chart account. You may not see that detail in your account until you have an appointment scheduled with a Baptist Health provider.

You will generally receive a response within 1-3 business days. Note that My Baptist Chart messages should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

On the Personal Information page in My Baptist Chart, you can update your address, phone number, email address, preferred name, and other personal details at any time so that your clinic always has the most up-to-date information in your record.

Your My Baptist Chart information comes directly from your electronic medical record at your provider's office. Ask your provider to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record after each visit.

• EHI Export generates your health information in computer-readable files (files that a computer or application can read).
• The files in this export are not designed to be read by humans; they are designed to be uploaded into other programs that can organize the information in the files into a readable form.
• The files in this export could be very large and could take Baptist Health several days to process.
• You may request a copy of your EHI Export through your My Baptist Chart portal or by contacting the Release of Information Office.
• To obtain a human-readable copy of your records, visit our Medical Records page.

My Baptist Chart offers personalized and secure online access to your medical records. It enables you to manage and receive information about your health. With My Baptist Chart, you can:

You need access to a computer or mobile device connected to the Internet and an up-to-date browser (such as Edge, Chrome, Firefox, or Safari).

You can also access My Baptist Chart in our Baptist Access App or the MyChart app App Store | Google Play Store.

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account must be accessed using that password. On mobile, the account may also be accessed using face ID or fingerprint login. Unlike conventional email, all My Baptist Chart messaging is done while you are securely logged in to our website or mobile app.

You can ensure that your account stays secure even if someone else has your username or password by turning on two-step verification. When this feature is turned on, you must enter a code that is sent to you by email or text message to log in to My Baptist Chart, in addition to using your username and password. Baptist Health in Jacksonville, Florida started using EPIC My Chart as their Electronic Medical Record (EMR), or Electronic Health Record (EHR), on July 30, 2022. Read more details.

When you log into MyChart, select your upcoming appointment and click get on the waitlist. If an earlier appointment opens up, we'll send you a message letting you know. You can either accept the new appointment time or keep your existing time. New appointments are given on a first-come, first-served basis, so it's important to respond to the message immediately.

Yes, your Baptist Health records and data will still be available in this new portal.

However, any self-published data will not be transferred. For instance, if you added any over-the-counter medication or allergy information to My Baptist Connect for your own record-keeping, this information will not transfer to My Baptist Chart.
