Our AgeWell patients are 65 years or older. Many need assistance with one or more of the following conditions:
- A decline in the ability to function or care for themselves
- Unsteady balance or at risk for falls
- Bowel or bladder problems (incontinence)
- Memory loss, dementia or Alzheimer’s disease
- Depression, anxiety
- Lack of appetite or unexplained weight loss
- Multiple medications and/or needs assistance with medication therapy
- Multiple ER visits or unplanned hospitalizations

AgeWell provides a combination of clinical and care partner support programs and staff training to help improve the quality of life and independence. These programs are offered on an inpatient, outpatient, emergency room, at home and community basis.
AgeWell Center for Senior Health
The AgeWell Center helps older adults; many facing challenging medical issues live more independent and productive lives by providing them with an enriched level of specialized primary care in one central location. AgeWell provides peace of mind to those who are caring for an older loved one by promoting healthy aging through a comprehensive plan of medical care.
NICHE and AgeWell Advanced Care Unit (ACU)
Baptist Beaches is a designated NICHE Level 1 hospital, which demonstrates a commitment to the continued progress in achieving patient-centered care for older adults. Baptist Beaches has developed an Advanced Care Unit (ACU) to make hospital stays easier for older adults.
AgeWell Emergency Care
Designed to meet the special needs of elderly patients who may have complex medical issues, our Senior Emergency Services at Baptist Beaches is staffed by a team specially trained in the needs of geriatric patients.
AgeWell Eldercare Advocate Program
The Eldercare Advocate Program helps seniors over the age of 60 to safely transition to their home environment after a hospital stay. The advocates help coordinate non-skilled care needs such as meals, transportation, home making, core services and other home and community-based services.
AgeWell Training, Education and Outreach Programs
AgeWell offers gerontology training and staff development programs at Baptist Health and for professional staff in the community. The programs include:
Ageless Wisdom, a highly interactive, two hour program for caregivers and other community members.
Delirium Management and Prevention Program, an intensive online program that distinguishes delirium from depression and dementia. Training uses the Confusion Assessment Method (CAM) to assess, manage and prevent delirium. The online program is followed up with in-person training and competency testing.
AgeWell Speaker’s Bureau, a service which offers speakers for all age-related topics to community organizations, caregiver coalition workshops, faith-based organizations, and other conferences. Call us at 904.202.4622 to request a speaker.
