Anne Yates, APRN has been a board-certified family nurse practitioner since 2010 and RN since 1982. Anne’s experience includes 15 years in the inpatient setting as an RN and 13 years as an APRN in outpatient settings of Internal Medicine and Home-based primary care. Anne had the privilege to provide home-based primary care for geriatric patients over the last 9 years in Age Well’s house call program. Anne is bringing 9 years of experience back into the clinic. Her passion includes patient education on up-to-date guidelines for the geriatric population and focus on health promotion, disease prevention and disease intervention for acute and chronic conditions.

About Me

I am a Jacksonville native and moved to the Beaches after Nursing School in 1982. My oldest daughter lives close by with my two grandsons with who I am usually with, if not at work. Our favorite place to visit is the happiest place on earth, Disneyworld. My Youngest daughter lives in Denver CO and returns home to visit 3-4 times a year. My philosophy of care is to remember patient care is a privilege and to build a positive relationship on trust, mutual respect, communication and understanding over time.

I have worked for Baptist Health as an RN at the Beaches hospital and APRN in Primary care. As a native of Jacksonville, I am aware of other Health Systems in town and I chose Baptist Health. Mainly because I was an RN at the Beach where I live and took care of patients in our community I wanted to be able to provide primary care to help prevent hospitalizations and recurrent hospitalizations in our community.

I chose BH in 2014 when Baptist Beaches hospitalist started a program that aligned with my goals of becoming an APRN in the Beaches community. The care program to prevent re-hospitalizations developed into home-based primary care for the elderly in the community. I have worked for BH for almost 9 years and recommend BH Primary Care, Specialists and Hospitals because of the improved coordination of care that improves patient outcomes.
