Under the direction of a licensed professional, the Care Coordination Assistant provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care. This role plays an important part in supporting smooth care transitions for patients and families.
Key Responsibilities
Provide non-clinical and clerical support to the Care Coordination and Pediatric Social Work teams.
Assist with coordination of care and discharge planning activities.
Support patient and family care transitions across the continuum of care.
Communicate with internal departments and external agencies as directed.
Maintain accurate documentation and assist with data entry in the electronic medical record.
Coordinate meetings, referrals, and follow-up appointments as needed.
Ensure timely completion of assigned administrative tasks.
