Job Summary
Baptist Medical Center is looking for an Care Manager to join our team in Jacksonville, FL. This is a Full-Time days opportunity located at Baptist Metro Square campus.
Care Manager, Job Responsibilities:
- High level screening, assessing and planning for an identified high risk group of patients in the ER, Critical Care and other services as needed.
- Collaborate with the attending, ancillary, professional and technical staff to coordinate patient care.
- Screen patient for admissions or discharge utilizing Utilization Review criteria.
- Participate in oversight of resources in critical care and emergency services area.
Experience Requirements:
- 3-5 Years Case Management Experience
- 3-5 Years Emergency Department Experience
- 3-5 Years Discharge Planning Experience
- 3-5 Years Critical Care Experience
Education & Credential Requirements:
- Bachelor of Science in Nursing required
- Master of Arts/Science preferred
- Basic Life Support (BLS) required
- Licensed Registered Nurse required
