Job Summary

Baptist Health is currently looking for a Decentral Internal Medicine Pharmacist to join our Pharmacy team at Baptist Medical Center located in Jacksonville, FL. This is a full-time, day shift opportunity, working 7:00 am - 3:30 pm, Monday through Friday and every fourth weekend.

As an Internal Medicine Pharmacist, this role provides clinical pharmacy coverage for both Medical-Surgical and Progressive Care units, collaborating closely with physicians, nurses, and interdisciplinary care teams to optimize medication therapy. On a daily basis, the pharmacist reviews medication orders, monitors patient responses, performs medication reconciliation, and provides evidence-based recommendations to improve patient outcomes. Throughout the week, they manage complex medication regimens for patients with acute and chronic conditions, assist with transitions of care, and support antimicrobial stewardship and medication safety initiatives. The pharmacist also serves as a medication expert and educational resource for healthcare providers while contributing to quality improvement efforts across the inpatient setting.

A strong background in clinical pharmacy experience and/or a completed PGY1 is preferred. Candidates must posses a Pharmacist license in the State of Florida in order to be hired into this position.

As a Decentral Internal Medicine Pharmacist other duties will include:

  • Participate in daily interdisciplinary rounds with hospitalists, nurses, and healthcare professionals.

  • Review and evaluate medication treatment plans for safety, efficacy, and cost-effectiveness.

  • Monitor patient progress and collaborate on discharge planning and medication education.

  • Enter medication orders accurately and review technician work for precision.

  • Serve as a resource for healthcare professionals, offering expert drug therapy consultation.

  • Supervise pharmacy technicians and contribute to departmental committees.

  • Monitor patient therapy including pharmacokinetics services and intervene when necessary.

Education & Credential Requirements:

  • Bachelor of Science Required

  • Registered Pharmacist - State Board of Pharmacy

  • Licensed Pharmacist - DOH
