Job Summary

Baptist HealthLead Care Manager is looking for a Registered Nurse, at Baptist Medical Center. This is a Full Time Day shift opportunity in the Heart One Department at Baptist Jacksonville - South campus.

As a Lead Care Manager, you are responsible for:

• Coordinate and approve staff PTO/Holiday requests.
• Complete daily Case Management assignments (on-call assignements/weekend assignment requirements).
• Performing as a clinical resource and liaison to Case Management staff.
• Collaborating with multidisciplinary team members.

Education Required

Bachelor's Degree - Nursing

Education Preferred

Master's Degree - Nursing

Experience

• 3-5 Years Case Management Experience Required

License and Certification

• Certified Case Manager, CM (from ABOHN) Preferred
• Licensed Registered Nurse Required
