Job Summary
Baptist Medical Center is looking to add a Licensed Clinical Social Worker in our Social Services Department at Baptist Medical Center in Jacksonville, FL. This is a full-time days opportunity.
Licensed Clinical Social Worker, Job Responsibilities:
- Assess psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community
- Provides discharge planning, care coordination, arranges home health and DME.
- Provides counseling, assists with placement/transfers, provides referrals to community services.
Experience Requirements:
- 5 years Social Work experience
- Case Management experience
Education and Credential Requirements:
- Master's Degree (MA/MS)
- Licensed Clinical Social Worker (LCSW)- FL Department of Health
** A valid Florida LCSW is required for this role, registered interns will not be considered**
