Job Summary
AgeWell is seeking a Nurse Practitioner or Physician Assistant to join our growing practice in Jacksonville, FL. The selected Advanced Practitioner Provider would serve as a link between inpatient and outpatient care as a member of the Baptist Health team.

- Master’s degree
- 2 or more years of working experience in Family Practice
- Certified and Licensed APRN
