Job Summary
The provider is responsible for the care and treatment of patients, including diagnostic and therapeutic interventions. Duties include delivering high-quality medical care, maintaining accurate and timely documentation, adhering to established standards of care, fostering positive patient and community relations, and ensuring regulatory compliance. The provider will diagnose, treat, and help prevent mental health disorders. Residents and Fellows are encouraged to apply.
This position specializes in serving the geriatric population. The provider will work collaboratively with patients, their families, the AgeWell team, and behavioral health professionals to deliver comprehensive, patient-centered care. The role follows a hybrid model, with a combination of in-office and remote workdays.
