Job Summary

Baptist Health is seeking a dedicated and compassionate Registered Nurse, Case Manager to join our team. This is a PRN opportunity and plays a key role in supporting patients throughout their hospital stay by ensuring coordinated, high-quality care.

Registered Nurse, Case Management, Job Responsibilities

• Provide comprehensive care coordination for hospitalized patients.
• Conduct high-risk screenings and assessments to identify patient needs.
• Arrange and document post-acute care services, ensuring smooth transitions of care.
• Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.

Experience Requirements:

• 1 year case management experience

Education & Credential Requirements:

• Associate of Science in Nursing required
• Bachelor of Science in Nursing preferred
• Cardiopulmonary Resuscitation Certification (CPR) Required
• Licensed Registered Nurse Required
