Job Summary

Baptist Health is seeking a Master's Level Social Worker to join our team. This is a PRN Days opportunity working at our Baptist Downtown campus.

This Social Worker will be a part of an interdisciplinary care team with the goal to enhance well-being by helping meet the needs impacting a person's life through psychosocial support and connection to appropriate resources. There is a focus on alleviating barriers and lessening the burden of medical conditions.

Social Worker II, Job Responsibilities:

• Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
• Provides discharge planning, arranges home health and DME
• Provides counseling and assisting with placement/transfers
• Provides referrals to community services

Experience Requirements:

• 1-2 years Social Work experience
• 3-5 years Discharge Planning experience
• Home Health experience

Education and Credential Requirements:

Masters of Social Work required
