As the only community-owned, locally governed, not-for-profit health system in Northeast Florida, Baptist Health exists to build a happier, healthier community. Baptist Health’s Strategic Investment Program directs resources to organizations leading impactful work in the priority areas identified through the 2025 Northeast Florida Nonprofit Hospital Partnership Community Health Needs Assessment (CHNA).

Our Priorities

Each of Baptist Health’s six hospitals identified priority community needs to address during the 2026–2028 CHNA cycle. Strategic investments support initiatives aligned with these priorities and serve each hospital’s geographic service area. Additional details can be found in each hospital’s community health implementation plan.

Our Process

Organizations that meet our eligibility guidelines are encouraged to connect with us. All strategic investments are by invitation and begin with a phone or virtual meeting.

Baptist Health prioritizes investments that expand equitable access to essential services across Baker, Clay, Duval, Nassau, and St. Johns counties—particularly for populations experiencing disproportionate barriers to health and well-being. As such, general operating expenses not tied to program expansion are outside the scope of this program.

Funding requests should clearly describe how support will:

  • Serve additional individuals
  • Reach new communities
  • Strengthen program capacity beyond current operations

We prioritize community-based organizations and partnerships that demonstrate:

  • Cultural competence
  • Strong community trust and relationships
  • Capacity to deliver measurable, high-impact programs

Eligibility Guidelines

Investments & Reporting

  • Small Strategic Investments

    Small strategic investments support timely, community-driven efforts that advance health and well-being, including pilot initiatives, emerging needs or targeted program expansion.

    Investment size: Up to $15,000

    Process:

    1. Initial virtual meeting
    2. Invitation to apply (if aligned)
    3. Rolling application submission
    4. Internal review and decision (typically within two weeks)

    Organizations new to Baptist Health funding are generally first considered through a small strategic investment.

  • Large Strategic Investments

    Large strategic investments support high-impact initiatives that expand access to care and address priority community health needs.

    Strategic investments size: Over $15,000

    Process:

    1. Initial virtual meeting
    2. Submission of concept narrative and preliminary budget
    3. Invitation to submit full application (if aligned)
    4. Staff review and refinement
    5. Formal review and due diligence
    6. Final decision by the Community Health & Well-Being Committee

    The Committee, composed of representatives from each of Baptist Health’s six hospital boards, meets five times per year to consider funding requests.

Strategic investment recipients are required to submit two reports:

  • CHNA Data Report

    Due: November 15

    Includes:

    • Number of individuals served
    • Other key outputs tied to CHNA priorities
    • Data covering the period of October 1 – September 30 (regardless of your strategic investment period)
    • Client stories and/or survey results

    Note: Reporting must align with this timeframe regardless of the organization’s strategic investment period. Baptist Health reserves the right to modify these guidelines and procedures at any time.

  • Impact Report

    Due: One month after the end of the 12-month funding period

    Includes:

    • Progress toward stated goals and objectives
    • Implementation successes and challenges
    • Lessons learned
    • Client stories (optional)
    • Financial summary of expenditures

    Note: Reporting must align with this timeframe regardless of the organization’s strategic investment period. Baptist Health reserves the right to modify these guidelines and procedures at any time.
