Job Summary
Baptist Health is seeking a dedicated and compassionate Registered Nurse, Case Manager to join our team at the Jacksonville campus. This is a PRN opportunity and plays a key role in supporting patients throughout their hospital stay by ensuring coordinated, high-quality care.
Registered Nurse, Case Management, Job Responsibilities
- Provide comprehensive care coordination for hospitalized patients.
- Conduct high-risk screenings and assessments to identify patient needs.
- Arrange and document post-acute care services, ensuring smooth transitions of care.
- Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.
Experience Requirements:
- 1 year case management experience
Education & Credential Requirements:
- Associate of Science in Nursing required
- Bachelor of Science in Nursing preferred
- Cardiopulmonary Resuscitation Certification (CPR) Required
- Licensed Registered Nurse Required
