Job Summary

Baptist Medical Center is looking to add a Licensed Clinical Social Worker in our Social Services Department at Baptist Medical Center in Jacksonville, FL. This is a PRN opportunity.

Licensed Clinical Social Worker, Job Responsibilities:

• Responsible for high-level screening, assessment, and care planning for an identified high-risk patient population in the Emergency Department.
• Collaborates with attending physicians, resident physicians, nursing staff, and ancillary, professional, and technical personnel to coordinate the delivery of high-quality, individualized, and cost-effective patient care.
• Utilizes Utilization Review criteria, admission guidelines, and discharge criteria to determine the appropriateness of patient admissions and discharges.
• Implements interventions to enhance patient safety, quality of care, and clinical outcomes.
• Maintains ongoing communication with patients and families, medical staff, caregivers, and third-party payors to establish priorities, coordinate services, and develop and implement goal-directed care plans.
• Promotes efficient patient throughput and works to reduce inappropriate hospital readmissions.
• Participates in the oversight of resource utilization in critical care and emergency services areas to support efficient, cost-effective care delivery.

Experience Requirements:

• Social Work Experience
• Case Management Experience
• 3-5 years Discharge Planning Experience
• 3-5 years Case Management Experience

Education and Credential Requirements:

• Master's Degree (MA/MS)
• Licensed Clinical Social Worker (LCSW)- FL Department of Health

** A valid Florida LCSW is required for this role, registered interns will not be considered**
