Job Summary
Baptist Health is seeking a compassionate and motivated Social Worker II to join the team at Wolfson Children’s Hospital. This Full-Time evenings/weekends position plays a vital role in supporting pediatric patients and their families by addressing psychosocial needs and ensuring access to essential services and resources.
Key Responsibilities
- Assess patients’ psychosocial needs to support emotional well-being and optimal functioning.
- Provide discharge planning, including coordination of home health services and durable medical equipment (DME).
- Assist with patient placement and transfers, ensuring smooth transitions of care.
- Offer referrals to community resources to support ongoing care and family needs.
