Job Summary

Baptist Health is seeking a compassionate and motivated Social Worker II to join the team at Wolfson Children’s Hospital. This Full-Time Days position plays a vital role in supporting pediatric patients and their families by addressing psychosocial needs and ensuring access to essential services and resources.
Social Worker II, Job Responsibilities:
  • Assess patients’ psychosocial needs to support emotional well-being and optimal functioning.
  • Provide discharge planning, including coordination of home health services and durable medical equipment (DME).
  • Assist with patient placement and transfers, ensuring smooth transitions of care.
  • Offer referrals to community resources to support ongoing care and family needs.
  • Support utilization review activities to promote effective and appropriate care delivery.
Experience Requirements:
  • 1-2 years Counseling Experience Required
  • 1-2 years Experience in Child Welfare Services Required
  • 1-2 years Social Work Experience Required
  • 1-2 years Hospital Experience Required
  • Less than 1 year Experience in Behavior Health Setting Required

Education & Credential Requirements:

  • Master of Social Work required
  • Licensed Clinical Social Worker Preferred
  • Driver's License Preferred
