The Baptist Hospital at Home program supports healing by providing compassionate, 24/7 hospital-level care where you feel most at easeāin your own space.
Recover in familiar surroundings while still being closely monitored by a care team of doctors, nurses and community paramedics providing continuous support during recovery.

How the program works
If your doctor determines you need hospital-level care but are well enough to recover safely at home, this program may be recommended. Conditions we commonly treat at home include pneumonia, chronic obstructive pulmonary disease (COPD), cellulitis, urinary tract infections (UTIs), and more.
If you are eligible and choose to participate in the program, we will begin the transfer process. Your care team will meet with you to explain your care plan and get you ready to be sent home for immediate treatment. Any needed medical equipment will be delivered to your home.

Insurance information
Although some health insurance plans, including Medicare, cover hospital at home services, we will help you check with your insurance provider to confirm coverage. The Baptist Hospital at Home program is billed the same as a hospital stay.

As a Hospital at Home patient, you may receive:

A personalized care plan developed by physicians, nurses, and other healthcare professionals to fit your needs.

At least two daily in-person visits from a community paramedic, plus a daily virtual visit with a physician.

Hospital-level services at home, depending on your specific needs, may include labs, EKG, IV infusions, medication management, oxygen/respiratory care, and imaging.

24/7 remote monitoring with two-way audio/video connectivity; all equipment provided.

Meet Mr. Johnson: Recovering with Hospital at Home
Mr. Johnson (a fictional patient) visited the emergency department with a prolonged cough, shortness of breath, wheezing, and fatigue. After being diagnosed with a respiratory infection, Mr. Johnsonās doctor recommended he continue his recovery at home through Baptist Hospital at Home. The care coordination team quickly arranged everything he would need to safely recover at homeāincluding transportation from the hospital, meals, and all the medical equipment required for 24/7 virtual monitoring.
At home, Mr. Johnson wears a small device that tracks his vital signs and uses a tablet to connect with his care team. Every day, he has a virtual visit with a physician, and a community health paramedic visits twice daily to check his progress and provide any hands-on care he needs.
His care plan is personalized specifically for him and coordinated by a team of doctors, nurses, therapists, and other healthcare professionals. Depending on his needs, he also has access to labs, EKGs, IV medications, oxygen therapy, imaging, pharmacy services, nutrition support, physical therapy, occupational and speech therapy, and social work and case management services.
After spending just a few days in the program, Mr. Johnson successfully completed his inpatient care at home. The care team removed all special equipment and helped transition his follow-up care to his primary doctor, ensuring he continues to recover safely and comfortably.
With Baptist Hospital at Home, patients like Mr. Johnson get hospital-level care without leaving the comfort of home.
