Lung Screening Request

A low-dose CT lung screening is a quick, safe and non-invasive way to detect problems early, when treatment is most effective. This screening is covered by Medicare and most commercial insurance plans. For those who do not meet coverage criteria, a self-pay option is available for $170 ($110 for the scan and $60 for the radiologist fee).

Please consider this important screening if you:

• Are between 50 and 77 years old
• Have a 20 pack-year history of smoking or more (this means 1 pack a day for 20 years, 2 packs a day for 10 years, etc)
• Are a current smoker or have quit within the last 15 years
