Request an Appointment Confirmation
Thank you for submitting your request for a new patient, geriatric assessment. AgeWell staff will be contacting you by phone within the next business day to schedule a convenient date and time for your first and second visits to the AgeWell Center for Senior Health.
To help you prepare for your first visit, please print out and review the information on this page. To complete your assessment, it’s critical that you complete the checklist below in advance and bring it with you to the visit.
First Visit: Getting to Know You (up to 3 hours)
First, you’ll meet your AgeWell Care Coordinator, who will get to know you by asking a few questions about your overall physical and emotional health, your typical diet and daily activities. Depending on your physical abilities, he/she may ask you to walk and perform other light tasks to assess your mobility and balance.
You will also meet with our pharmacist to review your current medications.
Finally, the Geriatrician will talk with you about your health, how you are feeling and how well you are functioning. He/she will perform a physical exam and may order laboratory tests, if needed. Our center has an on-site laboratory for your convenience.
Preparing for Your First Visit
To help you get organized for your first visit, we’ve prepared the following checklist to help you get organized for your visit.
Health Risk Assessment
You will receive the online assessment form once your appointment has been scheduled.
Medications
Please gather ALL of your medications in a bag, including prescription medications, over-the- counter medicines, vitamins and herbal supplements. Please bring the actual bottles. Also ask your pharmacy to print a complete list of medications you have taken over the past 12 months and bring that.
List of Doctors
Make a list of the names and phone numbers of all your doctors and the reasons you see each of them.
Health Information:
Please write down the following:
- Any surgeries or hospitalizations you have had in the last two years
- All diagnoses of illnesses or conditions your doctors have told you about
- Allergies (medicine related and all other)
- Health screenings and immunizations you have had
- Diseases in your family
- Advance Directives
If you have completed advance directive forms, please bring with those you to your first visit.

Who Should Attend the First Visit
If possible, please bring someone with you who is involved in your life and helps you with health-related matters. It could be your spouse, family member, friend or caregiver.
Comfort
Please wear comfortable clothing and walking shoes to your visit.

If you would like interpretive services during your visit, please call the center at 904.202.4600 and we will make the arrangements.
