Job Summary
Baptist Health is currently looking to add an experienced and compassionate Family Advocate Champion to our social services team for Wolfson Children's Hospital located in Jacksonville, FL. This is a Part-Time position of 20 hours per week working Monday through Friday. The primary role of the Family Advocate will be to reduce family stress by providing services and resources with Care Navigation, Emotional Supports, Educational Tools and Advocacy/Empowerment. The Family Advocator will bring their lived experience as the caregiver for a medically complex child or youth with special health care needs to the care team which will enhance overall care of the patient and their families. The Family navigator will use their experience and knowledge to assist with planning, problem solving, education and empowerment.

As a Family Advocate Champion, you will be responsible for:
- Maintain communication and linkages with families and children with complex health care needs to provide support, exchange informatin, and obtain input on program actions.
- Reviews and evaluates information from families and professionals to determine possible courses of action that may improve service delivery systems.
- Provide resources for camps, childcare, respite, sibling and family programs, hospice support and other support and therapy groups.
- Prepares written reports, organize meetings.
- Assists the center with the development and implementation of quality improvment initiatives and PDSA cycles, collaborate in community based participatory research (CBPR), representing Wolfson in CHA/CMMI meetings, webinars, link Parent Group to other regional support groups and community outreach.
- Participating in formal and informal advocacy endeavors.
- Provides assistance to the project director and practice transformation facilitator in the development, delivery, and evaluation of activities.
