Baptist Medical Center is currently looking to hire an Inpatient and ED Social Services Manager. This is a Full-Time Day Shift opportunity.
Our Inpatient and ED Social Services Manager at the Baptist Jacksonville campus will be working in a hospital setting that will be the first line of supervision and include managing the daily operations of the team that focus on patients with health-related social needs and other social complexities. The position leads, mentors, educates and provides guidance to the team while working closely with other stakeholders to support the overall care coordination strategy while meeting its goals and objectives of departments and health system. The role works closely with leadership across the health system, collaborating with the Inpatient Case Management leadership, hospital and ED leadership, BPP leadership, and the quality specialist to develop strategies and implement best practices and workflows that support both socially and medically complex patients within the hospital and ED. The position also collaborates with community partners with a shared goal of connecting patients to appropriate resources. This role's span of control includes Inpatient Social Services, ED Care Coordination and community engagement efforts.

Inpatient and ED Social Services Manager, Job Responsibilities:
- Providing leadership to and supervision of Inpatient and ED Social Services staff.
- Oversees daily operations.
- Collaborate with Inpatient Case Management, Hospital, ED and BPP leadership as well as community partners and stakeholders.
- Develop strategies and implement best practices and workflows that support both socially and medically complex patients within the hospital and ED
- Leads, mentors, educates and provides guidance to the team.
