Job Summary
Baptist Medical Center is looking to add a Social Worker I to join our team. This is a Full-Time opportunity.
As a Social Worker I, you will be responsible for:
- Assess psychosocial needs.
- Provides discharge planning, arranges home health and DME.
- Assists with placement and transfers.
- Provides referrals to community services.
- Assists with utilization review activities.
**A Bachelor's degree in Social Work is required**
