Job Summary
Baptist HealthMaster’s Level Social Worker (Social Worker II)PRN, day shiftinpatient social work support across all Baptist Health campuses is currently seeking a to join our team on a basis. This role will provide, working as part of an interdisciplinary care team.
As a Social Worker II, you will play a vital role in enhancing patient well-being by addressing psychosocial needs, reducing barriers to care, and connecting patients with appropriate community resources. Your work will help alleviate the burdens associated with medical conditions and support holistic patient care.
Key Responsibilities:
Assess psychosocial needs and provide supportive services to enhance patient functioning and emotional well-being
Deliver care coordination, psychoeducation, and referrals to community resources
Advocate for continued access to services that support recovery and quality of life
Collaborate with healthcare providers to ensure a patient-centered approach
Qualifications:
Master’s degree in Social Work (MSW) is required
