Job Summary
Baptist HealthMaster’s Level Social Worker (Social Worker II)Full-Time, Day Shift is seeking a dedicated and compassionate to join our team. This is a opportunity working at our Baptist South campus.
In this role, you will be a key member of an interdisciplinary care team, working to improve overall well-being by addressing the psychosocial needs that impact patients' lives. The focus is on reducing barriers to care and easing the burden of medical conditions through emotional support, education, and resource connection.
Key Responsibilities:
Assess psychosocial needs and deliver services to enhance patient functioning and emotional well-being
Coordinate care to ensure continued access to support and services
Provide psychoeducation to patients and families
Connect individuals with appropriate community resources and referrals
Qualifications:
Master’s degree in Social Work (MSW) is required
