Taylor Sousa, APRN, is a board-certified family nurse practitioner with clinical expertise in treating patients across the lifespan in both inpatient and outpatient settings. She enjoys educating patients on health promotion and disease prevention and is especially passionate about heart health. She is compassionate and caring and believes that shared decision-making between the provider, patient, and family results in the most optimal outcomes.

About Me

I was born and raised in St. Augustine, Florida. However, I have done most of my schooling and lived in the Jacksonville area for the last few years. In my spare time, I enjoy spending time with family and friends, cooking, working out, bike riding, and just being outdoors.

I am board-certified in providing care to patients of all ages with various conditions, but I have a special interest in caring for those with cardiac conditions. I believe strongly in preventative medicine and its ability to result in positive health outcomes. I treat each patient as if they were my family and strive to form excellent patient-provider relationships and provide care that places the patient at the center of their treatment plan. I firmly believe that patients should have the necessary information to be more proactive in the management of their health and that this results in more optimal outcomes.
