Job Summary
The Ambulatory Care Coordinator will interact with geriatric patients with the goal of impacting the quality and efficiency of care • This coordinator will work closely with AgeWell Center for Senior Health personnel in order to improve the quality of care and decrease the fragmentation of care that is experienced by patients • Focusing on the medically complex patients, specifically those discharged from the hospital, this coordinator will develop and implement a plan of care to determine the extent of needs, and identify potential problems that affect the patients' post-discharge care.Education Required
