Job Summary

Baptist Medical CenterBaptist Metro Square campus is looking for an Care Manager to join our team in Jacksonville, FL. This is a Full-Time days opportunity located at.

As an Care Manager, you will be responsible for:

• High level screening, assessing and planning for an identified high risk group of patients in the ER, Critical Care and other services as needed.
• Collborate with the attending, ancillary, professional and technical staff to coordinate patient care.
• Screen patient for admissions or discharge utilizing Utilization Review criteria.
• Participate in oversight of resources in critical care and emergency services area.

Education Required

Bachelor's Degree - Nursing

Education Preferred

Master's Degree - Nursing

Experience

• 3-5 Years Case Management Experience
• 3-5 Years Emergency Department Experience
• 3-5 Years Discharge Planning Experience
• 3-5 Years Critical Care Experience

License and Certification

• Basic Life Support (BLS) Required
• Licensed Registered Nurse Required
