Job Summary

Baptist Health is looking to add a Manager of Care Coordination to our team! Under the direction of the Director of Care Coordination, the nursing Manager of Ambulatory Care Coordination is responsible for the daily operations of the Ambulatory Care Coordination team focusing on medically complex patients, and serves as first line supervision and oversight of daily operations. The Manager leads, mentors, educates, and provides guidance to the team while working closely with other stakeholders to support the overall care coordination strategy while meeting its goals and objectives of Value Based Population Health. The Manager works closely with BPP leadership, collaborating with the Social Services Manager of Ambulatory Care Coordination, the Manager of Operations of the commercial plans, the clinical educator, and the quality specialist to develop strategies adn implement best practices and workflows that support BPP contractual arrangements for both socially and medically complex patients within our Medicare contracts.
