Social Services Manager

Job Summary

Baptist Medical Center is hiring a Social Services Manager. This is a Full-Time Day shift opportunity and will be located at Baptist Metro Square.

Under the direction of the Director of Care Coordination, the Manager of Social Services is responsible for the daily operations of the Ambulatory Care Coordination Social Work team focusing on socially complex patients, and serves as first line supervision and oversight of daily operations

As a Social Services Manager, Job Responsibilities:

• The Manager leads, mentors, educates and provides guidance to the team while working closely with other stakeholders to support the overall care coordination strategy while meeting its goals and objectives of Value Based Population Health

• The Manager works closely with BPP leadership, collaborating with the nursing Manager of Ambulatory Care Coordination, the Manager of Operations of the commercial plans, the clinical educator, and the quality specialist to develop strategies and implement best practices and workflows that support BPP contractual arrangements for both socially and medically complex patients within our contracts

• The manager also collaborates with community partners and Social Responsibility with a shared goal of connecting patients to appropriate resources.

Experience Requirements:

• 3-5 Years Leadership Experience

Education and Credential Requirements:

• Master of Social Work (required)
• Doctorate of Arts/Science (preferred)
