Job Summary

Baptist Health is seeking a Master's Level Social Worker to join our team. This is a Full Time Days opportunity working at our Baptist Metro campus.

This Social Worker will be a part of an interdisciplinary care team with the goal to enhance well being by helping meet the needs impacting a person's life through psychosocial support and connection to appropriate resources. There is a focus on alleviating barriers and lessening the burden of medical conditions.

As a Social Worker II, you will be responsible for:

  • Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
  • Providing care coordination
  • Providing psychoeducation
  • Providing referrals to community services

Social Worker II Experience:

  • 1-2 years Social Work Experience
  • Home Health Experience

**A Master's degree in Social Work is required**
