Job Summary
Baptist Medical Center Jacksonville is looking for an MSW to join our highly skilled team Palliative Care department. This is a PRN Day shift opportunity working Tuesday and Wednesday with at least 15 Saturdays per year.
This position requires a high performing MSW who can practice both independently and as collaborative team member with the palliative IDT and our hospital partners. The fast paced work environment demands flexibility, proactive problem-solving and excellent communication skills. The MSW plays a key role in our value based care model to normalize goals of care for patients dealing with life-limiting illnesses. The MSW will also support the Palliative LCSW and introduce consults to patients and families, address advance directives and be knowledgeable about statutes and physician orders, provide disease education on the progressive nature of illness and ensure that the care provided is concordant with the goals established. In addition, they are expected to assess psychosocial needs, assess coping and support systems and provide services that enhance patient functioning, emotional well-being and access to services and resources in the community.
A Master's Degree in Social Work is required.
*FL Registered Clinical Social Worker Intern in good standing with Florida's Department of Health required.
