Baptist Health is looking for a Social Services Team Lead at our Baptist Jacksonville campus. This is a Full Time Days opportunity.
The Social Services Team Lead will assess psychosocial needs and provide services that enhance patient functioning, emotional well-being and continued access to services and resources in the community. Provide discharge planning, arrange home health and DME, assist with placement/transfers, provide referrals to community services.

As a Social Services Team Lead, you will be responsible for:
- Social work resource for team members and care coordination program.
- Provide guidance through challenging situations.
- Promote problem-solving/critical thinking skills among team members to be utilized upon new challenges/barriers.
- Assist with interviewing and hiring staff members who will enhance the care coordination program’s ability to meet its goals and objectives.
- Coordinating orientation and training for newly hired staff. through personal mentoring or assigning a mentor, and monitoring the new staff member’s progress towards established expectations.
