Job Summary
Baptist Medical Center is looking to add a Wellness Coach Dietitian to our Population Health Transformation Department at Baptist Metro Square, located in Jacksonville, FL. This is a full-time opportunity working Monday to Friday from 8:00am-4:30pm.
As the Wellness Coach Dietitian, you will be responsible for:
- Conducting health risk assessments and anthropometric measurements
- Develop meal plans and provide nutritional education for clients
- Implementing nutrition care plans and medical nutrition therapy
- Communicate effectively in-person, telephonically, and virtually with clients and multidisciplinary teams
- Field referrals with a focus on pre-diabetic and diabetic patients
- Must be willing to travel locally and work early morning and evenings as needed.
- Must be a Registered Dietitian and licensed in the State of Florida.
