Baptist Medical CenterHealth Coordinator is looking for a to join our team. This ia Full Time Days opportunity working at our Baptist Metro Square campus.
The Health Coordinator supports nurses in ambulatory care coordination by assisting with the care of medically complex patients and members of contracted health plans. This role includes telephonic outreach to patients, helping to coordinate care and provide ongoing education. Responsibilities also include assessing patient goals, promoting healthy lifestyle choices, connecting patients with community resources, and supporting the execution of care plans as determined by the primary care provider.
As a Health Coordinator, you will be responsible for:
- supporting nurses in ambulatory care coordination
- assessing patient goals
- promoting healthy lifestyle choice
- connecting patient with community resources
- supporting care plans under the direction of the primary care provider

- 1-2 years Experience coaching wellness needs
License and Certification
- Cardiopulmonary Resuscitation Certification (CPR) Required
