Social Worker I - Full Time Days - Baptist Jacksonville

Job Summary

Baptist Health is seeking a Bachelor's Level Social Worker to join our team. This is a Full-Time Days shift opportunity. The schedule with be M-F 8-hour shifts with no weekends required. 

This Social Worker will be a part of an interdisciplinary care team with the goal to enhance well being by helping meet the needs impacting a person's life through psychosocial support and connection to appropriate resources. There is a focus on alleviating barriers and lessening the burden of medical conditions. 

As a Social Worker I, you will be responsible for:

• Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
• Providing care coordination
• Providing psychoeducation
• Providing referrals to community services
