Your highest of quality professionals. In addition to rockstar doc Wallace, everyone from Candace/Dinah to PAs Mike & Cara. I had so much confidence in the screening with Cara at recent visit that there was no need to take up Dr Wallace's time. Keep up the great quality!! Candace has been super responsive in handling appts and my inquiries.

This essentially was my neurosurgery meet and greet. Send the survey back around in a few months when I have a true feel for things. Based on first impressions, the facility is clean with with an atmosphere that does not scream hospital system. Effortless check in process that I personally feel providers should use across the board. Friendly and helpful supporting staff. My provider was easy to communicate with and seemingly understood my concerns.

It was easy to make an appointment with Dr. Shaw and I came at the referral of another patient of his, Dr. Rey Pardo's mother in law. She was Dr. Shaw's patient and Dr. Pardo is my pain management doctor - I would only have seen Dr. Shaw because Dr. Pardo trusts him and referred him. I live in South Florida and have been a patient of Dr.. Pardo's for more than a decade (I used to live in Jax and could not find a comparable doctor when I moved to South Florida, so I drive up to see him). Hence when I came up to see Dr. Shaw, it was coupled with a visit to Dr. Pardo - I didn't know that I would, in fact, not be meeting Dr. Shaw at all that day, so I was disappointed about that. I had wanted to find out what surgery Dr. Shaw would recommend, either in the near or far future, given my lumbar issues. I did not get that question answered, nor did I really get any firm response to what Dr. Shaw (as a preferred neurosurgeon for complex spinal issues) would do, if anything. His PA was lovely, a very nice woman. But I'm still in the dark about my future surgically as I was before I came in. So, all in all, a very nice complex, nice people and easy appointment, but the entire purpose of the visit seems to have failed, to me.

I???m happy to say that your staff was exceptional. Leslie took me back and she was personable and thorough with her questions. Kara came in shortly and she spent much of our time together explaining the results of my scan. I never felt rushed and I was so informed when I left. I must say this office visit scored a perfect 10!

I met with the NPR and she was informative and provided me her analysis of my issue. I had the XRAY performed that day and am still waiting for feedback on what is showed. I am also waiting to schedule an MRI, so we can have all the information needed to assess my neck pain. I would like to schedule the MRI as soon as possible. On the progress notes from the appointment, the medications listed are incorrect. I only take Lisinopril and the other that are listed I have taken them in the past but NO LONGER take them. I shared that with the NPR. It just needs to be updated.

I feel most fortunate that I found Lyerly Neurosurgery and more specifically Dr Andrew Shah just in the nick of time ! Everyone at Lyerly that I have had interaction with from the front desk back to Dr Shah???s staff including Erica, his MA, Heather Bean, his ARNP, Kara and Melissa, both ARNPs are so very professional, knowledgeable and compassionate individuals. Dr Formoso cares deeply about helping his patients and it is extremely evident. He makes you feel relaxed and less anxious. He takes the time to answer your questions and explain the options of what could work best for you. I would absolutely recommend him.

I did not feel valued. I did not feel the warmth as i have felt in the past. If I was a new patient I would not return. The people was not warm and receptive. Not friendly just cold. They seemed unhappy with their job. This is a first time experience for me alt this establishment. People come here sick and in pain. A little empathy makes a difference. It hurts no one to smile or say a kind word. No one seemed concerned or cared.

I was seeking this appointment as a second opinion. Appreciated the visit with NP Kara but did not have enough info lot make an informed decision about next steps. Will need to see Dr Shaw After more imaging. As expected. I had already seen a NP at UF neurosurgery so feel I am in same place at this point. It was very challenging to make it from the parking lot to the dept relying on my cane and my husband. By the end of the day, I was in severe pain. Such an inconvenience is troubling for future care. Very different than UF locations including the downtown location. Very troubling

Everyone was genuinely very nice in the office. Every part of the visit was efficient, from start to finish. The provider followed up in a timely manner as promised. The receptionist at the front were so very friendly and informative. When my husband and I met with the nurse practitioner she was kind, understanding and most importantly (especially these days) she gave us her time while listening intently. I greatly appreciated that. She didn't rush us along or make me feel I was imposing on her busy schedule. I highly recommend Lyerly to all people that need their services!
